You’re used to treating injuries in the field, making do with whatever resources you have to hand. When you treat a Wound that has an ongoing effect until it is healed, make a Recall Test. If you succeed, the ongoing effect is ignored for the rest of the battle.
In addition, you do not need specialist medical facilities to attempt surgery. You can attempt surgery in the middle of a battle, though this is an Exacting Dexterity Test requiring 8 successes, and each Test costs an action.
